How to find a participating Delta Dental provider:

  1. Click here for the Delta Dental provider search.
  2. Delta Dental Premier Plan – Under “Select your Network” choose either the Delta Dental PPO or Delta Dental Premier network.

Pro Tip:  The Premier Plan allows you to use any dentist you wish, but you will receive the greatest benefit by using a dentist who participates in the PPO network.

How to find a participating DeltaVision provider:

  1. Click here for the EyeMed Select network search.
  2. Enter your city or zip code, and choose your network to generate a list of vision care providers near you.

How to use Teladoc:

  1. Click here for directions to create your Teladoc account.
  2. To begin using your Teladoc services, simply download the app and set up an account.
