Dental Plan

Delta Dental of Iowa

Delta Dental of Iowa offers several different options to choose from so you can select the one that best meets your needs.

Highlights of the Delta Dental of Iowa plans include:

  • Preventative Prime Benefit Summary
    • Preventive care covered at 80% when visiting an in-network provider
    • No coverage limit for routine and preventative care
    • Coverage for cavity repair and restoration of decayed teeth 
  • Preferred Prime Benefit Summary
    • Preventative Care covered at 100% when visiting an in-network PPO provider
    • Routine Care such as cavity, tooth extraction and routine oral surgery covered at 50% when visiting an in-network PPO provider
    • Coverage for endodontic and periodontal work – waiting periods apply
  • Platinum Prime Benefit Summary
    • Preventative Care covered at 100% when visiting an in-network PPO provider
    • Routine Care such as cavity, tooth extraction and routine oral surgery covered at 80% when visiting an in-network PPO provider
    • Coverage for endodontic and periodontal work – waiting periods apply

DeltaVision Insight Preferred Plan

DeltaVision Insight Preferred Plan Benefit Summary

    • $10 eye exam with dilation at an in-network provider
    • $130 allowance for frames or contact lenses
    • $10 prescription lenses
    • Discount on Lasik or PRK vision correction

Pro Tip:  The DeltaVision plan is only available if you are enrolled in a Delta Dental of Iowa dental plan.

VSP Vision Plan

VSP Individual Vision Plan Benefit Summary

    • $15 eye exam at an in-network provider
    • $150 allowance for frames or contact lenses
    • $25 prescription lenses
    • Discount on Lasik or PRK vision correction

Pro Tip:  You do not need to be enrolled in a dental plan to enroll in the VSP vision insurance plan.

Accident & Sickness Plan

ASHIP is an Accident and Sickness Hospital Indemnity plan provided by Chubb Insurance.  This plan was designed to provide cash benefits to help pay the deductible and out of pocket expenses not covered by your health insurance plan. Cash payments are sent directly to you.  You are not required to use them to pay the provider. Your coverage will begin the 1st of the month following enrollment.

How to Submit a Claim

  1. The Accident and Sickness Plan Claim Form must be submitted within 20 days of the occurrence.
  2. Answer each item on page 1 and sign the form where indicated. Your Member ID is your Student ID number.
  3. Your policy number is 9908-37-36
  4. You may submit a claim form by mail, email, or fax.

Avoid waiting rooms! Teladoc gives you 24/7/365 access to doctors and mental health providers with unlimited visits at $0 copay. Get the care you need at a time and place that works for you. You can meet with the providers through the Teladoc app or website. Once a student enrolls, Teladoc benefits are available to spouses and dependents at no additional cost.
